Collection of information

Usually information will be collected directly from you when you enquire about or use our services. This could be through email, phone call, virtual or in person sessions, Google form, website, or forms sent from our client management system. We will endeavour to explain the purpose of the data being collected at the time of collection.

In some cases, where you have given consent, we may collect information from third parties, such as other health professionals, supervisors or others whom we need to contact in the course of providing our services.

What information is collected

The sort of information we collect will depend on the reason for our engagement with you, but we will only ever collect information which is necessary for us to carry out our services. The types of personal information that we may collect, and hold, include:

  • Your name, date of birth, and contact information such as your address, email address, phone number, and emergency contact details;

  • Information about your history or current situation that is relevant to us providing our services to you;

  • Any additional information you provide, such as when you contact us, or when you complete an intake form;

  • Session notes/records of appointments;

  • A record of any queries you have made or communications you have with us; and

  • Any other information that you authorise us to collect.

Use of personal information

We may use personal information for the following purposes:

  • To provide our services and to communicate with you, or those you have authorised us to communicate with, regarding these services;

  • To respond and follow up on any queries, complaints or requests you might have made;

  • To maintain and improve the services we deliver, including the quality of our services (for example feedback, enquiries, and other communications)';

  • To send you a monthly newsletter or other resources if you have opted in to being added to our mailing list (you can unsubscribe any time);

  • To share your personal information with third parties where you have consented for that information to be shared with them;

  • Any other purpose you have consented to or have authorised.

Sharing or disclosing information

Generally information will only be shared or disclosed with your consent. We may disclose your personal information as needed, and only the details that are necessary, to:

  • Our professional advisors (e.g. insurers, accountants, consultants, supervisors)

  • To your other service providers (with consent);

  • To the Police or other emergency services if we consider that this is necessary to keep you or others safe;

  • To any person where disclosure is required or authorised by law; and

  • To any other person or entity to whom you have consented us to disclose your personal information.

Storage, Privacy and security

We take reasonable steps to ensure your personal and health information is protected against loss, unauthorised access, use, modification, disclosure, or other misuse.

We hold personal information in various formats, including electronic and physical forms (paper files). We use third party providers to store and process our data (Squarespace, Splose, Halaxy, Google Workspace). Electronic information is password protected with two factor authentication. Sensitive health information is only held within secure client management systems, Splose and Halaxy, which are designed for this purpose. Any paper files are kept in a locked cabinet.

Personal information will be held as long as necessary to achieve the purposes for which we collected the personal information, and otherwise as required by law. We are required to hold health information for 10 years under the Health (Retention of Health Information) Regulations 1996 from the last time we provided services to you. After such time, we will arrange for the secure destruction of your personal information.

Accessing and Correcting Your Information

You can request to access or correct your personal information by contacting us at rebecca@chronicresilienceot.com .

When you contact us, please provide evidence of who you are and outline the specific details of your request (e.g. the personal information, or the correction, that you are requesting).

If the information held by us is incorrect, inaccurate, incomplete, or not up to date, then you may ask us to correct the information and we will either correct the information or make a record of your comment, as we think appropriate.

Privacy Concerns, Questions or Complaints

If you have any questions about this Privacy Policy and/or our handling of personal information or you believe that we have at any time failed to keep one of our commitments to you to handle your personal information in the manner required by the Privacy Act or the Code, you can lodge a written complaint to rebecca@chronicresilienceot.com

This Privacy Policy is governed by New Zealand law. Your personal information will be collected, used, stored, and disclosed in accordance with this Privacy Policy and New Zealand law. The courts of New Zealand have non-exclusive jurisdiction.

Chronic Resilience offers supervision, mentoring, occupational therapy and training across New Zealand and Australia. Some services are available worldwide. Digital worksheets and resources are also available through this website’s shop page.

We collect, hold, use, and disclose personal information about you in the course of providing our services.
